You are hereby notified that on the 18th day of July, 2008, Jerry Michael Welch, filed in this Court a Petition for Adoption of Chad Robert Sinkovich, a minor, whose date of birth is 08/14/1996, and for change of the name of the minor to Chad Robert Welch. This Court, located at 700 Adams Street, Toledo, Ohio, will hear the petition on the 8th day of January, 2009 at 10:00 a.m. It is alleged in the Petition, pursuant to R.C. 3107.07, that the consent of George Thomas is not required due to the following: The parent has failed without justifiable cause to communicate with the minor for a period of at least one year immediately preceding the filing of the adoption petition or the placement of the minor in the home of the petitioner. The parent has failed without justifiable cause to provide for the maintenance and support of the minor as required by law or judicial decree for a period of at least one year immediately preceding either the filing of the adoption petition or the placement of the minor in the home of the petitioner.
