Petition for Adoption
and On Application to
Place Child
Notice must be served not less than 20 days before the date of hearing
To: Sherri Sims (mother of Otisha and Diamond), Rachel Ragen (mother of Justin and Jerri), Otis Brookins (father of Otisha), whose whereabouts is unknown and cannot with reasonable diligence be ascertained, through service by publication in Toledo Legal News.
You are hereby notified that an Application to Place Child was filed on the 1st day of November, 2006, and a Petition for Adoption was filed on November 1, 2006 by Earlene Crisp for the adoption of Otisha Chavera Brookins (DOB 6/8/93) and Justin Lajay Sims (DOB 3/31/99) and Jerri Elizabeth Sims (DOB 2/9/00) and Diamond Holmes (DOB 8/23/03).
A hearing has been set for April 12, 2007 at 10:00 a.m. This Court is located in the Lucas County Court House, 700 Adams Street, Suite 200, Toledo, Ohio 43604.
It is alleged in the Petition, pursuant to R.C. 3107.07, that the consent of Sherrie Sims, Rachel Ragen and Otis Brookins is not required due to the following:
You have failed without justifiable cause to communicate with the minor and/or provide for the maintenance and support of the minor as required by law or judicial decree for a period of at least one year immediately preceding either the filing of the adoption petition or the placement of the minor in the home of the petitioner(s).
